Here's an example of what I see and get to repair on an all to regular basis. This damage has come from someone using a Big Frigging Hammer to beat on the end of a steering column shaft to remove the steering wheel. The nut was left in place but you can see that the threads were ruined regardless. The nut was beat on so much that the threads just sheared off. Five minutes with a steering wheel puller available at any auto parts place could have prevented this damage. Yes, I can repair the damage...
